Social Worker- Full Time 0.7 FTE (28 hours per week)
Description
The Social Worker as part of a multi-disciplinary team is responsible for providing social case work, case management, family and group work services to patients of the FCFHT.
Common Responsibilities:
2.1 Working in a professional manner that demonstrates integrity, respect and valuing the diversity of staff, patients and individuals.
2.2 Working in a manner that preserves confidentiality and seeks to minimize risk.
2.3 Being flexible in their practice to meet the needs of the patient.
2.4 Maintaining competence, and where applicable, a professional license to practice.
2.5 Contributing to the Team’s efforts to secure and maximize resources for current and new programs, services and activities.
2.6 Contributing to the Team’s activities to collect, analyze and report on data and relevant information, as well as participate in research and program evaluation. .
2.7 Contributing to the Team’s practice of orienting and training new staff and staff development activities.
2.8 Supporting or supervising student and volunteer placement programs.
2.9 Participation in Team activities including meetings, committees and special events.
2.10 Incorporating and strengthening collaborative and interdisciplinary teamwork.
2.11 Working during regular and extended hours of operation in locations identified by the Team.
2.12 Prepare quarterly or as required statistical reports for the MOHLTC or as required by the FCFHT.
2.13 Maintain current knowledge of policy manuals and procedures.
Primary Responsibilities:
3.1 Provide psycho-social assessment of patients.
3.2 Provide evidence based counseling services (individual, couple and family) or referral to address presenting issue.
3.3 Assist physician with referrals to secondary and tertiary health care centers.
3.4 Provide and participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services.
3.5 Act as an advocate for patients and their families in matters affecting health.
3.6 Identify and support linkages to community, volunteer and professional services.
3.7 Liaise with other agencies and providers, i.e., hospitals, CCAC, etc...to provide case management and negotiation of adequate follow-up resources in the community.
3.8 Maintain complete and accurate medical records and participate in chart reviews.
3.9 Be a consultant for team members on methods to manage psychosocial and mental health problems.
3.10 Provide education for patients and families on how to live with chronic health conditions.
3.11 Other duties as assigned.
Qualifications - Required:
5.1 Masters and/or Undergraduate degree in Social Work from a recognized university.
5.2 Registered or eligible for registration with Ontario College of Social Workers and Social Service Workers (OCSWSSW)
5.3 Three to five years experience as a Social Worker with patients and families in a health setting.
5.4 Demonstrated ability to work in a multi-disciplinary setting.
5.5 Experience in program development, implementation, monitoring and evaluation.
5.6 Excellent communication, interpersonal, time management and conflict resolution skills.
5.7. Proficiency in use of computers.
